Although routes from the US that originate in Miami, MIA, Boston, BOS, New York, JFK, do have direct flights to Rome, they have more frequent flights that route through Malpensa.

For those not interested in renting a car in Milan, you can always hop on the train, the “Malpensa Express” and take that directly to Milano Centrale.
If you don’t take the train, there is a bus shuttle service that makes a stop at Linate Airport, and then Milano Centrale. From the central train station you can connect to popular destinations like Venice, Bologna, Verona, Genoa, Cinque Terre, and Rome.
